This questionnaire is intended to provide you with an opportunity to evaluate the effectiveness of the Internship Program. We believe the students who complete an internship are the most qualified to provide us with suggestions for improvement and we would appreciate your candid responses to these questions. The information you provide will be confidential and will not be released to your employer. If the Faculty Advisor is contacted, results will be summarized and no reference will be made about the students(s) submitting the evaluation.
